How to fill out the WH-347 certified payroll, column by column
The WH-347 is the Department of Labor's optional form for the weekly certified payroll on Davis-Bacon and Related Acts projects. You can use it or any format that contains the same information (29 CFR 5.5(a)(3)(ii)). This is how the pieces fit together.
Header
Project name and number (or contract number), project location, wage determination number, the certified payroll number (1 for the first week you work on the project, then in sequence), the week ending date, your business name and address, and whether you're the prime or a subcontractor. Tick “final” on the last payroll for the project.
Columns 1–3: who and what
The worker's name and an individually identifying number — not the full Social Security number on weekly transmittals; the last four digits or your own worker ID is enough. Column 2 is J (journeyworker) or RA (registered apprentice). Column 3 is the classification of work actually performed, as listed in the wage determination. A worker who did two kinds of work gets a line for each.
Columns 4–5: hours
Hours each day, straight time on the top line and overtime below, and the weekly total. On federal contracts overtime is hours over 40 in the workweek (40 U.S.C. 3702); California public works also apply daily overtime (Labor Code 510).
Column 6: rates and fringe
6A is the hourly rate paid for straight time and overtime. 6B is the hourly credit for contributions to bona fide fringe benefit plans; 6C is fringe paid in cash. Together with the base rate they must reach the wage determination's base + fringe.
Columns 7–9: pay
7A is gross earned on this project; 7B is gross for all work that week (other projects included). Column 8 lists deductions for all work — tax withholdings, FICA, other (explain “other” in the remarks) and the total — and 9 is net pay.
Page 2: Statement of Compliance
Six statements, signed by the person who pays or supervises payment. Box 4 lists apprenticeship programs when apprentices are on the payroll; box 5 lists each fringe benefit plan and the hourly credit claimed for each worker. See the Statement of Compliance guide.
